Written by the Plenthera team (orthomolecularly trained). Scientifically reviewed in April 2026. Last updated: April 30, 2026.

Betaine HCl is not an enzyme but a hydrochloric acid supplement – for those who produce too little stomach acid. With age (own stomach acid production decreases) or after prolonged use of antacids, protein digestion can falter, for which stomach acid is essential. At Plenthera, you'll find betaine HCl as a standalone capsule or combined with pepsin – the stomach acid-dependent protein enzyme. WARNING: DO NOT use with stomach ulcers, gastritis, or reflux symptoms.

Which betaine is right for you?

Form Best for
Betaine HCl 600 mg Stomach acid support
+ pepsin More complete protein digestion
TMG (separate) Methylation – different application

What is betaine?

Betaine HCl (betaine hydrochloride) is a hydrochloric acid supplement that supports stomach acid levels. Not an enzyme, but relevant for stomach acid-dependent digestion. Often combined with pepsin.

Stomach acid level is an underestimated aspect of digestion. Sufficient stomach acid is essential for: protein breakdown (activates pepsin), mineral absorption (calcium, iron, magnesium, B12), and protection against food pathogens. Our own stomach acid production decreases with age — from about 60 years old, a significant portion of the population produces suboptimal stomach acid (hypochlorhydria).

Betaine HCl supplementation is a direct stomach acid supplement. Taken with a meal (especially protein-rich), it temporarily lowers the pH in the stomach — similar to natural stomach acid. The starting dosage is usually 600 mg, to be built up until a slight warmth is felt in the stomach — that is a sign that you are taking enough.

CRUCIAL CAUTION: betaine HCl is CONTRAINDICATED in case of stomach ulcers, gastritis, GERD/reflux symptoms, or when using NSAIDs or corticosteroids. If in doubt: consult a medical professional beforehand, in accordance with the advice of the NVWA.

Science and EFSA status

Clinical research on betaine HCl is limited. In hypochlorhydria research, it is functionally used under the guidance of orthomolecular therapists. EFSA has not recognized any claims.

The betaine forms at Plenthera

Betaine HCl 600 mg

Standard dosage. Starting point for those who want to build up.

Betaine HCl + pepsin

Premium combination — pepsin requires stomach acid to be active.

TMG (trimethylglycine, different category)

Methyl donor, different application. Not for stomach acid purposes.

How to use betaine?

Simple basic approach for betaine HCl:

  1. Start with 1 capsule (600 mg) with a protein-rich meal.
  2. Gradually increase until you feel a slight warmth in your stomach — that is the optimum.
  3. Decrease again to just below the warmth threshold.
  4. For stomach ulcers/gastritis/reflux: DO NOT use — can worsen symptoms.
  5. For NSAIDs or corticosteroids: consult a doctor beforehand.
  6. Especially relevant for protein-rich meals (meat, fish, legumes).

Myths and misconceptions about betaine

Myth 1: "Everyone with digestive issues has low stomach acid."

Incorrect. High stomach acid (reflux) and low stomach acid can cause similar symptoms. If in doubt: medical diagnosis.

Myth 2: "Betaine HCl treats reflux."

Incorrect and risky — can actually worsen reflux. Reflux treatment requires a different approach.

Myth 3: "High dosage works faster."

Incorrect and can cause irritation. Build up gradually.

Combining betaine with other supplements

Betaine HCl is often combined. Popular combinations:

  • Pepsin: stomach acid-dependent protein enzyme. Classic combo.
  • Digestive enzymes: for broad coverage.
  • Probiotics: for gut flora.
  • Bone broth: gastrointestinal whole food.

Frequently asked questions

What is betaine HCl?

Hydrochloric acid supplement for stomach acid support. Not an enzyme.

Do I have low stomach acid?

Symptoms: bloating after protein, fatigue, mineral deficiencies. Medical test (Heidelberg or bicarb test) confirms.

How much betaine HCl per day?

Start with 600 mg, build up to slight warmth.

When to take betaine HCl?

With a protein-rich meal.

Betaine HCl for elderly?

Potentially relevant — own stomach acid decreases.

Difference between betaine HCl and TMG?

Betaine HCl: stomach acid. TMG: methyl donor, different application.

Betaine HCl for stomach ulcer?

DO NOT use — can worsen symptoms.

Does betaine have side effects?

Stomach complaints with overdose or incorrect use.

Betaine + pepsin?

Classic combo — pepsin requires stomach acid.

Betaine HCl experiences?

Variable — can help for those with low stomach acid, not for others.
